Send the following documents by post with tracking (e.g., a simplified registered mail, LetterPack Plus) to your department office. Your score information will be sent by post (LetterPack Plus) after being issued.

<What you need to send to us>

・Application Form

・Examination Admission Card

・Copy of your Identity Confirmation Card
(ID card issued by a public institution, such as a Driver’s License [if your address is written on the back of the card, you need to copy the back, too], Passport [including your photo page and information on bearer], Health Insurance Card [including the side where your address is written, and make sure to black out your private information, such as the Number, the Kigo記号, Bango番号, 枝番, insurer number保険者番号, and QR code], or Student ID Card if you are student of UTokyo.)

・Self-address LetterPack Plus as a return envelope
　* Please prepare a red LetterPack Plus envelope. The blue LetterPack Light is not acceptable.
　* Only a self-addressed return envelope is acceptable.

After Application Submission Date (see below), submit the following documents to our department office.

・Application Form

・Examination Admission Card

・Identity Confirmation Card

After Score Information Issuing Date (see below), visit our office again to receive your score information. Bring your Examination Admission Card and your Identity Confirmation Card with you.
